New Patients

Nurse explaining paperwork to a patientWe want to make your first visit with us pleasant and efficient. There are a few things that you can do to help us:

  • Registration, information release and health history forms are available on this website; please print them and fill them out before coming to our office. Click here for forms.
  • Insurance information should be brought with you. Click here for more information on insurance and billing.
  • An accurate list of medications that you are currently taking is important. If there is any question, bring the medication bottles with you.
  • If you are scheduled for testing, please follow the pretest instructions very carefully, especially those that relate to fasting and medication schedule. Click here for more information on tests.


At your visit, a medical assistant will ask you change into a gown, weigh you, check your blood pressure and obtain an EKG before the doctor sees you. Depending on the problem and at your convenience, we may ask you to stay longer than your scheduled time to do further tests so that we can give a better assessment of your condition that day. Therefore, please allow at least 60 minutes for your visit.
